These Terms of Service ("Terms") govern your use of LYVN Health, Inc.'s website and telehealth platform. By using our services, you agree to these Terms.
Telehealth services
LYVN facilitates asynchronous and synchronous consultations with independent licensed healthcare providers. LYVN is not a medical practice and does not provide medical advice directly. Treatment decisions are made solely by your assigned provider.
Eligibility
You must be 18 or older (or the age of majority in your state), located in a state where we operate, and provide accurate medical information. Providers may decline treatment if not clinically appropriate.
Payments
Fees are disclosed at checkout. Subscriptions and starter programs are billed as described on the pricing page. You authorize us and our payment processor to charge your selected payment method.
Compounded medications
Some prescriptions may be fulfilled as compounded medications from licensed pharmacies. Compounded products may not be FDA-approved as finished drug products. Your provider will explain risks and alternatives.
Limitation of liability
To the fullest extent permitted by law, LYVN is not liable for indirect or consequential damages. Our total liability is limited to amounts you paid in the twelve months preceding the claim.
Governing law
These Terms are governed by the laws of the State of Delaware, without regard to conflict-of-law rules.
